PRIVACY NOTICE
Protecta Insurance is collecting information to evaluate whether they will provide insurance cover. The information collected will be held by Protecta Insurance at 110 Symonds Street, Auckland, the insurer Virginia Surety Company, Inc.(VSC) of Unit 3, Level 2, 73 Manchester Street, Christchurch and agents of these entities. You have certain rights to and correction of this information. Any correction or enquiry you wish to make can be directed to Protecta.
Protecta and VSC will use this information to provide a quote, consider your application for cover, for analytical purposes and where cover is accepted, to manage or settle your claims. To do this they may provide information to third parties such as other insurance companies, reinsurers, external claims data collectors, and other parties required by law. Where you provide information about other persons, you must have their consent to do this and provide it on their behalf. Where the information contains material facts, the disclosure of the information is mandatory. If you fail to provide the information requested, it may result in us not being able to provide you with the insurance or your insurer VSC may be able to avoid the insurance retrospectively.
YOUR DUTY OF DISCLOSURE
We are bringing to your attention what you must know about your duty of disclosure to us, Protecta, as managing agent, and VSC as the insurance company underwriting this insurance.
As part of this application we are asking various questions of you. You have a duty at law when answering them to provide us honest and complete answers, and tell us everything you know and that a reasonable person in the circumstances of this application should know. You are not only required under this duty of disclosure to disclose such information at the commencement of cover, you are also obliged to disclose such information at any time during the period the insurance is current including at the time the renewal of insurance is offered.
You answer the questions for yourself and anyone else covered by the insurance contract, because you apply on their behalf and with their consent. If you breach the duty of disclosure, we may cancel the insurance contract or reduce the amount we pay for a claim. If fraud is involved, we will treat the insurance contract as if it never existed and pay you nothing.

All information provided relevant to this insurance proposal is true and correct in every respect;
The declarations made and this insurance proposal shall be the basis of this contract;
I / We agree to accept the Insurance Contract subject to the terms and conditions contained therein;
I / We authorise VSC and Protecta to give to or obtain from other insurers or insurance advisors or financial institutions any information relating to insurance held by me / us or any claim made by me/us.
I / We understand that:
I / We are obligated to advise VSC and Protecta of any information in the future which may be material to this insurance proposal as it has considered acceptance based on what I / We have told VSC and Protecta.
Protecta Insurance has the right to accept or decline insurance cover on any vehicle model and or driver/rider (where applicable) I / we have proposed for insurance cover.).
